Dhyc case management

All residents receive an orientation and assessment during the first two weeks of their placement, after which a treatment plan is developed and they are integrated into the various appropriate programs. Residents are assigned a case manager as the individual’s contact person and coordinate daily and personal needs and crisis intervention.

 

Case management is the liaison for all in-coming contacts concerning the residents.
